Thank you for your interest in serving as a Teen Helper in Children's Ministry. This form is intended to be completed by a teen interested in serving with Children's Ministry AND their parent together.
Teens who have completed 5th grade and above are welcome to serve a minimum of once a month in Sunday School classrooms under the supervision of trained and background-checked adult leaders.

Security questions - the tough ones!
Have you ever abused a minor or engaged in any conduct that could be regarded as child abuse or neglect (see Appendix A of the Meadowbrook Church Child Protection Policy)?
Have you ever been convicted of, or pleaded guilty to, or are charges pending concerning any felony?
For those applications who are over the age of 18, we will conduct a background check. Do you have any reason to believe that such a background check on you would disclose any negative information?
Do you have a health issue or history of emotional illness that would currently place children, other workers or yourself at risk?
If your answer to any of the above questions is yes, please explain in a written letter, seal and submit to the MBC office c/o Background Checks.
1. I desire to model Christ-likeness in all my behavior, especially as I serve in Ministry to children or youth at Meadowbrook Church. I am to demonstrate my love for the Lord Jesus, my gratitude for His grace and forgiveness, and to testify to the power of the Holy Spirit working in me.
2. I desire to demonstrate respect, loyalty, patience, courtesy and maturity to all children and adults without regard to race, religion or culture.3. Whenever possible, I will avoid being along with a child other than my own.4. I will no abuse any child or youth either physically (striking, spanking, shaking or slapping), verbally (humiliating, degrading or threatening) or sexually (including inappropriate touching or exposure).5. I will not smoke, use or possess tobacco products in the presence of MBC youth or children.6. I will not use, possess or be under the influence of alcohol or illegal drugs at MBC or at MBC sponsored events.7. I will not possess weaponry of any kind in the presence MBC youth or children.8. I will not tolerate or use profanity in the presence of MBC youth or children.9. I will report any suspected abuse or neglect of a child or youth according to MBC's Child Protection Policy.10. I understand that any violation of this code may be grounds for dismissal as a volunteer.
SCRIPTURE We believe the Scriptures, both Old and New Testaments, to be the inspired Word of God, without error in the original writings, the complete revelation of His will for the salvation of men, and the Divine and final authority for all Christian faith and life.
GOD We believe in one God, Creator of all things, infinitely perfect and eternally existing in three persons, Father, Son and Holy Spirit.GOD THE FATHER We believe in God the Father, creator of all things visible and invisible, who is true holiness, eternal power, infinite love, and absolute truth.
JESUS CHRIST We believe Jesus Christ is true God and true Man, having been conceived of the Holy Spirit and born of the Virgin Mary. He died on the cross a sacrifice for our sins according to the Scriptures. Further, He arose bodily from the dead and ascended into heaven, where at the right hand of the Majesty on High, He is now our High Priest and Advocate, and in the person of the Holy Spirit, He indwells the believer.HOLY SPIRIT We believe the ministry of the Holy Spirit is to glorify the Lord Jesus Christ, and during this age to convict, regenerate, seal, indwell, guide, Instruct and empower for life and service.MANKIND We believe man was created in the Image of God but fell into sin and is therefore spiritually dead, and only through regeneration by the Holy Spirit can salvation and spiritual life be obtained.SALVATION We believe the shed blood of Jesus Christ and His resurrection provide the only ground for justification and salvation for all who believe and only such as receive Jesus Christ are born of the Holy Spirit and thus become children of God.
ORDINANCES We believe water baptism and the Lord’s Supper are ordinances to be observed by the Church during this age. They are, however, not to be regarded as means of salvation or prerequisites for church membership. (This church shall practice believer’s baptism by immersion.)
THE CHURCH We believe the true Church is composed of all such persons who are regenerated by the Holy Spirit through saving faith in Jesus Christ, are united together in the Body of Christ of which He is Lord and Head and are commissioned for the God-given task of proclaiming the gospel to every person. Only those who are thus members of the true Church shall be eligible for membership in the local church. Every local church has the right under Christ to decide and govern its own affairs.CHRIST’S RETURN We believe in the personal and imminent return of our Lord Jesus Christ, and that this has a vital bearing on the personal life and service of the believer.JUDGEMENT We believe in the bodily resurrection of the dead; of the believer to everlasting blessedness and joy with the. Lord, and of the unbeliever to judgment and eternal separation from God.
